Join our team at Chestnut Woods Rehab as a Social Worker.We are seeking a Full Time LSW/LCSW- In OfficeOffering $12k sign on bonusSame Day Pay!Pay Depends on Experience !Responsibilities of Social Worker:Evaluate patients (existing, new and prospective) based on their needs and limitationsAddress patient concerns and goals while maintaining constant communication with the patient as well as a case managerBuild rapport with patients and their familiesCoordinate care with PT departmentsDevelop care plans for patientsQualifications of Social Worker:Primary series of the COVID-19 vaccine requiredMust be able to work full-time hours and be flexibleBachelors degree required. Masters degree is strongly preferred.Minimum three (1) years' experience as a social worker in a healthcare-related setting.
