And Qualifications
- The Wound and Ostomy Center nurse is a licensed professional
responsible for planning and providing care for patients with
complicated wounds, and/or any patient who has a potential skin
care problem, and/or patients with a new or well established
ostomy, fistula, and/or percutaneous tube.
- Serves as patient advocate in order to assist the patients in
achieving an optimal level of wellness and independence.
- The Wound and Ostomy Center nurse will consult with other
members of the health care team in the management of acute and
chronic wounds, and/or ostomies.
- Under the guidance of the clinical supervisor of the Wound and
Ostomy Center established policies and procedures, the incumbent
performs the following functions:
- Assess and provide treatment to patients of the Wound and
Ostomy Center. Documents patients understanding of procedures
before treatment and receives signatures on informed consent,
including risks and hazards, to ensure patients understanding and
avoid liability.
- Assists the Provider in obtaining pertinent information from
all sources: history, diagnostic tests, patient and family.
Identifies pertinent medical/health issues and needs through
evaluation to establish appropriate priorities of care.
- Assess and document the condition of wounds and/or ostomy and
response to treatment. Identify medical / health issues based upon
evaluation and communicate concerns to Provider.
- Perform wound and/or ostomy treatments and therapeutic
procedures according to established department policies and
procedures and current standards of practice. Document physical
findings, treatments provided and plan of care in the patient's
record.
- Administer and document prescribed topical medications and
treatments in a safe manner.
- Triage scheduled patients to accommodate emergencies, with the
assistance of the provider and/ or clinical supervisor, to ensure
proper handling of priority cases.
- Coordinate outpatient/inpatient services in a cooperative
manner with other departments, with concern for patients and
families. Perform ankle brachial indexing, to include documentation
of values to serve as a diagnostic tool for providers.
- Employ proper isolation techniques to prevent cross
contamination between patients.
- Review lab values regularly. Notifies provider of abnormal
values to ensure safe therapy and prevent complications.
- Assist providers with debridements.
- Perform quality control testing of glucometer and performs
blood glucose monitoring on outpatient diabetics as ordered by
provider and reports any abnormal values to provider.
- Assist in arrangement of referrals, completes appropriate
documentation to provide for appropriate patient care intra and
inter-departmentally and with outside agencies.
- Communicates with patient and their significant others and/or
caregiver regarding plan of care.
- Identifies educational needs of patients/families, assists with
teaching, and documents in patient's record.
- Ensures confidentiality of employee, legal, client/patient
relationships.
- Maintains appropriate records to facilitate correct billing and
retrieval of charges.
- Demonstrates regard for the dignity and respect of all
patients, family members, visitors and hospital personnel.
- Provides and promotes effective and compassionate care for
patients and their families and addresses patient/family
adaptation, coping skills, and the need for crisis and other
interventions.
- Appropriately addresses age specific considerations and
documents in patient's record.
- Participates in departmental Quality Management/ Process
Improvement programs.
- Attends and participate in-service education programs and
department meetings to maintain state license.
- Attends and participates in department meetings.
- Participates in an appropriate role in disasters, fire drills,
fires and other emergency situations.
- Exercises care in the use and operation of equipment and
reference manuals. Performs preventative maintenance to ensuring
continued functioning of equipment. Reports malfunctioning
equipment to Manager of Wound and Ostomy Center. Maintains work
area in a clean, safe and organized manner in accordance with
infection control standards.
- Functions as a clinical resource person within the institution
and assists staff to practice in a manner which enhances patient
outcomes through role modeling expert nursing practices. Assists in
providing education materials and/or assists in programs to educate
staff, patients their families and works collaboratively with and
provides support to members of the multidisciplinary team.
- Serves on department and hospital committees. Actively
participates in quality management practices and with the
collection and organization of data. Assists in maintaining chart
reviews for quality purposes.
- Completes projects and commitments in a timely manner.
- Assists in maintaining par stock levels and performs routine
cleaning and preventive maintenance to ensure uninterrupted
functioning of the Wound and Ostomy Center29. Maintains current
state licensure and knowledge base on the prevention and treatment
of wounds/ostomies through attendance of educational programs,
seminars, conferences, reading journals, and participating in
committee or group programs.What qualifications you will need:
- Two to four years Comprehensive Wound and Ostomy
experience
- Current registration in the State of NH as a Registered Nurse
or compact Registered Nurse license.
- Basic Life Support (BLS) certification required.
- Certification as a Wound and/or ostomy specialist- WOCN, CWCN,
CSW, OMS or WCCCatholic Medical Center (CMC)is an acute-care
