primary care Conductoffice visits for routine and acute issues, and
home visits as needed AdministerAnnual wellness visits and health
risk assessments Carecoordination with other providers,
specialists, testing facilities, andagencies Populationhealth
leadership, in coordination with the Care Team Assistingthe care
team with phone triage and outreach Educatingpatients on their
